Dr. Saurabh Chandra, Chief Telehealth Officer at University of Mississippi Medical Center, presented a hybrid care model where inpatient care alternates between in-person and telehealth specialists to address the shortage of neurologists. This model maintains continuous specialty services, improves clinical outcomes, and avoids recruitment delays, demonstrated by successful implementation in a rural hospital.
Tammy Maher – Transforming CHF Management: High-Quality Virtual Care for Better Outcomes
Tammy Maher, Lead Nurse Practitioner for eHealth At Home at Rochester Regional Health, detailed the program, including its interdisciplinary team and patient-centered approach. The program enhances patient-provider relationships, uses advanced technology like TytoCare, reduces hospital readmissions, and provides comprehensive home management for chronic and acute symptoms.
Reducing readmissions for CHF patients
Rochester Regional Health ran an evidence-based project to reduce readmission rates for congestive heart failure (CHF) patients. They found that patients enrolled in the eHealth@Home program showed a 51% reduction in readmissions.
