Dr. Saurabh Chandra, Chief Telehealth Officer at University of Mississippi Medical Center, presented a hybrid care model where inpatient care alternates between in-person and telehealth specialists to address the shortage of neurologists. This model maintains continuous specialty services, improves clinical outcomes, and avoids recruitment delays, demonstrated by successful implementation in a rural hospital.

Tammy Maher, Lead Nurse Practitioner for eHealth At Home at Rochester Regional Health, detailed the program, including its interdisciplinary team and patient-centered approach. The program enhances patient-provider relationships, uses advanced technology like TytoCare, reduces hospital readmissions, and provides comprehensive home management for chronic and acute symptoms. 

Douglas Olivo, Director, Community Engagement (SDOH), Tribal and Rural Outreach Services at Oklahoma Complete Health, highlighted improving virtual care access and choice by addressing social determinants of health, enhancing broadband connectivity, and creating hybrid care models. He emphasized collaboration and innovative solutions to expand care options and improve outcomes in rural areas.

Abby Losinski, Director of Telehealth at St. Luke’s Health System, discussed innovative virtual care solutions addressing healthcare challenges like staffing and patient access. The centralized virtual care team offers new patient welcomes, urgent care partnerships, and specialized services, reducing ED visits and improving patient-provider relationships. Integration with the health system ensures seamless care and high patient satisfaction.

Dr. Sarah Schenck, Executive Director at ChristianaCare’s Center for Virtual Health, discussed its virtual primary care program. She highlighted the multidisciplinary approach, including Patient Digital Ambassadors, and the benefits of high-frequency, low-intensity care. This model reduces ER visits, improves chronic disease management, and enhances patient satisfaction and access.
